The role In these part-time roles, working in teams of two (2), you will be responsible for visiting Residential Aged Care facilities to conduct impartial funding assessments that ensure fair access to the optimal level of care and support for residents. As part of a positive, cohesive group of experienced practitioners, you’ll apply your combined expertise to an approved process that sees you:
Conduct desktop reviews of existing assessment documentation.
Conduct interviews with residents, carers and other relevant people
Collaborate with other practitioners in your team to accurately complete AN-ACC funding assessment tools
Combine your findings to assess the appropriate funding classification for restorative care / reablement.
Uniquely, direct care won't be part of your day-to-day!
Locations Located in Tweed Heads, these opportunities will see you visiting Aged Care facilities across the Tweed Heads and Gold Coast region. Please note that there will some travel reimbursements and potential accommodation for this position.

To be considered, you will need:
5 years’ experience in aged care or a related area of health care
Relevant Tertiary qualifications in Nursing, Physiotherapy or Occupational Therapy
Current unrestricted General Registration AHPRA
Demonstrated Australian aged care experience including working with people with dementia
Demonstrated experience conducting clinical assessments using a range of assessment tools
...and soft skills related to motivation, organisation and working independently and as part of a team.
