This position is working within our APM Futures program, based in Lake Haven commencing at 3 days per week (Wednesday to Friday) 8.30am – 5.00pm (hours could increase depending on caseload growth)
About the Role
APM Futures delivers highly individualised employment supports to NDIS Participants.
You will be motivating, mentoring and supporting personal growth, helping your participants to develop and establish their vocational identity, tailoring their supports to their individual requirements.
Provide hands on & highly consultative support to a small group of participants
Provide mentorship, encourage skills development and increase people’s capacity for work
Deliver person centred supports to participants, some transitioning from school to work
Provide on-site support once employment has commenced and assist with integration
Provide support and consultation to employers
Qualifications & Requirements

Eligibility to work in Australia
A current driver’s licence and a comprehensively insured vehicle
Have current First Aid and CPR certificate
Willing to complete a Criminal History Check
Able to obtain a Working with Children Check & NDIS Worker Screening Check
Able to successfully complete a NDIS Worker Orientation Module prior to commencement
Travel as required for service delivery and support of participants.
