RAS Home Support Assessor - Western Sydney

APM is excited to be part of the Federal Government Assessment and Referral Initiative for older Australians to access the care they need to remain living in their own home. We are committed to excellence in service delivery and looking for people with a passion for aged care and enabling better lives.

This is a permanent full time position covering the Western Sydney region.  

The role

As a Home Support Assessor you will be conducting in-home face-to-face home support assessments with clients on a daily basis, using a standardised national assessment process. You will work together with clients to develop individualised support plans to achieve wellness and healthy ageing goals, as well as matching and referring clients to appropriate support services. Ideally, the successful candidate will be required to service surrounding areas.

What we need from you

  • Preference for Certificate III in Aged Care/ Community Services or similar and/or
  • Aged Care experience as a carer, assessor or case manager
  • Passion for working with aged Australians and commitment to maximise their independence and autonomy
  • The ability to communicate clearly and form effective relationships
  • Confident in use of mobile technology (e.g. tablets and mobile phones)
  • Flexibility, adaptability and a strong work ethic
  • Professional presentation

Essential requirements

  • Eligible to work in Australia
  • Current driver's licence
  • Access to a comprehensively insured vehicle for work purposes
  • Criminal history check on request
