About the role

As a full time or part time Exercise Physiologist you will be providing high-quality treatment to our clients, in their own home, to ensure the provision of optimal health outcomes. This encompasses ensuring effective and efficient assessment, treatment planning, treatment implementation and evaluation in a multi-disciplinary setting.

In this role you will be…

• Ensuring appropriate delivery of quality Exercise Physiologist services tailored to the individual needs of the client and using a person-centred approach
• Conducting assessments and providing interventions to patients and clients in their own home
• Engaging with other Allied Health Professionals, GP’s and other Health Care Professionals to provide effective patient care
• Writing reports such as initial reports, progress and final reports and treatment management plans which meet the standards of funding bodies such as NDIS, DVA, Medicare and HCP Providers
• Able to reflect our practice values of Care, Respect, Integrity and Service Excellence in all that you do.
• Creating and maintaining effective relationships with Case Managers, GP’s, community centre’s and other referrers.

• Exercise Physiology degree, with accreditation with ESSA.
• Ability to work within a multi-disciplinary team; and ability to work autonomously in the community
• An enjoyment of working with the older adult and adult disability demographics
• Clean driver licence, access to own vehicle and comprehensive car insurance.
• Current Working with Children Check and NDIS Workers Screening Clearance, or able to obtain.
• Willing to undergo a National Criminal History check upon successful application.
