As a Home Support Assessor, supporting clients in the Loddon Mallee region, you will conduct face-to-face home support assessments with older Australians to enable access to the care they need to remain living independently in their chosen community. You will use a standardised national assessment process that focuses on wellness.

Working closely with the client, you will develop support plans with individualised goals. You will match and refer older adults to services which can assist them to achieve optimal wellness and healthy ageing.

  • A keenness to enable an individual’s empowerment and support their independence
  • Competent computer skills – you’ll be inputting information into a laptop
  • Ability to work autonomously – Assessments are done in person, at client’s own home
  • Ability to engage readily and build rapport and trust
