As a full time Occupational Therapist you will be providing high-quality treatment to our patients to ensure the provision of optimal health outcomes. This encompasses ensuring effective and efficient assessment, treatment planning, treatment implementation and evaluation in a multi-disciplinary setting. You will work across a defined region providing care for our elderly or disabled community in the comfort of their home with our home visiting service.
In this role you will be…
Ensuring appropriate delivery of quality Occupational Therapy services tailored to the individual needs of the client and using a person-centred approach
Conducting assessments relating to Occupational Therapy and management of patients and clients in their own home
Engaging with other Allied Health Professionals, GP’s and other Health Care Professionals to provide effective care to patients
Writing reports such as initial reports, progress and final reports and treatment management plans which meet the standards of funding bodies such as NDIS, DVA, Medicare and HCP Providers
Creating and maintaining effective relationships with Case Managers, GP’s, community centre’s and other referrers
