About the role As a Physiotherapist with Lifecare Homecare you will be providing high-quality treatment to our patients to ensure the provision of optimal health outcomes.
Lifecare Homecare provide allied health services to individuals in their home, with a focus on aged and disability care therefore we are seeking a Physiotherapist with an interest in working with the older adult and/or disability demographic. You will work across a variety of case presentations and clients across a defined region, Mandurah, providing care for our elderly community in the comfort of their home with our home visiting service.
In this role you will be……
Delivering quality physiotherapy services tailored to the individual needs of the client and using a person-centred approach
Conducting physiotherapy assessment and management of patients in a community setting in their own home
Using clinical reasoning, to create and implement appropriate treatment plans, which target the goals of the patient and with a particular focus on achieving functional goals and supporting independent living
Liaising with other Allied Health Professionals, GP’s, Surgeon’s, Sports Physician’s and other Health Care Professionals, as required, within and outside the team, to help provide holistic care to patients
Preparing initial reports, progress reports, final reports or treatment management plans, which meets a standard that complies with the various funding bodies, such as NDIS, DVA, Medicare and HCP Providers
