In these Local Area Coordination roles, working in Urraween Pialba, you will be responsible for assisting people with disability, their families, and their carers to build and pursue their goal to live their best life, exercise choice and control, and engage with the National Disability Insurance Scheme – Partners in Community.  

 

You will be responsible for: 

  • Collaborating with people with a disability to assess their eligibility for supports through an NDIS plan.
  • Connecting people with disabilities to supports within the local community 
  • Assist participants to understand and use the supports in their NDIS plan, ensure the plan is working well, and support participants apply for changes to their plan.  
  • Improving community knowledge of the NDIS and creating new opportunities to connect participants to services and support 
  • Meeting identified contractual KPIs

You will have: 

  • Demonstrated relevant experience and or lived experience  
  • Experience in supporting people to achieve collaborative outcomes 
  • Knowledge of the local community and the available options for social & economic participation for people with disability 
  • Demonstrated computer skills including capability with Microsoft Office 365 
  • Demonstrated ability to work within a performance based culture
  • Currently hold or able to obtain a current Working with Children Card and / or vulnerable people check; PEP and NDIS Worker Screening Clearance; and successful completion of NDIA entry level checks.
