In this role you will …
- Deliver high-quality Physiotherapy services tailored to each client's unique goals and circumstances.
- Conduct comprehensive assessments and provide evidence-based interventions within clients' homes and community settings.
- Collaborate with GPs, Allied Health professionals and key stakeholders to achieve the best possible client outcomes.
- Prepare clinical reports, treatment plans and recommendations that meet NDIS, DVA, Medicare and Home Care Package requirements.
- Build and maintain strong referral relationships with healthcare providers, case managers and community partners.
- Champion a person-centred approach while upholding our values of Care, Respect, Integrity and Service Excellence.
- Support clients to maximise their mobility, function, independence and quality of life.

Thinking about applying? Here's what we're looking for:
- Physiotherapy degree, with General AHPRA registration.
- Ability to work within a multi-disciplinary team; and ability to work autonomously in the community
- An enjoyment of working with the older adult and adult disability demographics
- Full working rights in Australia
- Clean driver licence, access to own vehicle and comprehensive car insurance.
- Current Working with Children Check and NDIS Workers Screening Clearance, or able to obtain.
- Willing to undergo a National Criminal History check upon successful application.
