Key Responsibilities
- Coordinate and guide therapy supports to achieve meaningful participant outcomes
- Collaborate with families, educators, therapists and service providers to create a seamless support experience
- Engage in multidisciplinary teamwork across Early Start Australia
- Build strong stakeholder relationships and advocate for participants' needs
- Support high-quality, person-centred service delivery and continuous improvement
- Contribute to positive outcomes that enhance independence, participation and quality of life
About You
- Experience within disability, health, education or early intervention sectors.
- Strong clinical reasoning and case management capabilities.
- Experience coordinating supports across multiple stakeholders.
- Excellent communication and stakeholder engagement skills.
- A commitment to delivering person-centred, evidence-informed services.
- A passion for empowering people to maximise participation and independence.
